The role of the Registered Nurse (RN) is to provide quality patient care according to the Community Clinic of Jefferson County and New York State Office of Mental Health Standards of nursing practice MHOTRS outpatient clinic, including administration of medications and treatments. RN must demonstrate ability to respond to medical and psychiatric emergencies in a prompt, precise and professional manner and be able to work in collaboration with other medical and non-medical professional staff to provide comprehensive, cost-effective care and to support a problem-solving approach to issues.
Assess, plan, document and evaluate patient care. Participates in quality improvement projects. Acts as a resource for nursing issues and keeps abreast of all aspects of assigned patient care. Contributes to multidisciplinary treatment planning by bringing a nursing focus to team collaboration and contributing to multidisciplinary treatment planning. Communicates effectively with medical team, Clinician team, Care Coordination Team, Crisis Team, and Clinic managers.
Interacts effectively with patients, families, and significant others. Reports directly to the Director of Behavioral Health Administration and will be assigned according to the needs of the clinic.
Essential Functions:
A. Core Registered Nurse Responsibilities (Applies to All Programs)
- Conducts nursing assessments and triages patients' physical and psychosocial needs.
- Collects and reviews relevant medical information (hospital records, medication lists, laboratory results) and communicates findings to prescribing providers.
- Develops and implements nursing plans of care under the direction and oversight of licensed prescribers.
- Administers medications and treatments as ordered by the prescribing provider.
- Documents all nursing services accurately and timely in the Electronic Medical Record (EMR), including follow-up tasking and reminders.
- Provides patient, parent, guardian, and foster parent education related to medications, treatment plans, and health conditions, in alignment with the plan of care.
- Maintains current knowledge of nursing standards, OMH regulations, and agency policies.
- Participates in quality improvement activities and compliance efforts across programs.
- Serves as a nursing resource to multidisciplinary staff.
- Maintains confidentiality and complies with HIPAA and agency privacy policies.
- Conducts nursing and health monitoring assessments in accordance with OMH 599 and clinic protocols.
- Supports the Open Access process by completing Health Monitoring Assessments to facilitate timely access to care.
- Assists prescribing providers with coordination of care, medication management, and follow-up.
- Assists with medication refills and manages the clinic nurse line.
- Conducts outreach to clients with gaps in medication management or who have not picked up prescribed medications, in coordination with providers.
- Assists with client treatment plans by providing nursing input and medical coordination support under the oversight of Psychiatric Nurse Practitioners or Psychiatrists.
- Collaborates with clinicians, care coordinators, and clinic leadership to support daily clinic operations.
- Manages clinic medical supplies and medication samples in accordance with policy.
- Conducts nursing assessments and health monitoring for TCRP residents in accordance with OMH 589 standards.
- Reviews medication administration records (MAR) and medication pill packs to ensure accuracy and compliance with agency medication policies.
- Coordinates medication administration and personally administers medications to TCRP residents as often as possible, consistent with OMH 589 standards, agency medication policies, and licensed prescriber orders.
- Administers and oversees medications within the TCRP program as ordered by licensed prescribers.
- Coordinates medical appointments and follow-up care for TCRP residents.
- Transports TCRP residents to and from medical appointments as clinically indicated and operationally necessary.
- Provides nursing consultation to direct care staff regarding residents' medical needs and medication administration.
- Participates in interdisciplinary team meetings related to TCRP residents.
- Provides on-call nursing coverage (1-2 weekends per month) to support 24-hour clinical program compliance.
- Communicates effectively with prescribers, clinicians, care coordination teams, crisis teams, and program leadership.
- Assists in daily planning with providers to support efficient clinical flow.
- Acts as a resource and support to agency staff regarding nursing and medical concerns.
- Participates in required interdisciplinary meetings and trainings.
