Shift Details: Monday‑Friday, cover one weekend per month and one major and two minor holidays per year. Towns served:
Shelton, Derby, Ansonia, Oxford, Seymour.

Job Description

• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self‑advocacy and independence.
• Utilize state‑of‑the‑art technology to document skilled services provided.

Qualifications

• Licensed Practical Nurse with a valid CT LPN license.
• One year of LPN experience;
  Homecare experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.
