Shift Detail: Cover Cheshire, Hamden, Prospect, Wolcott Mon - Friday. One weekend per month coverage with optional comp days. Cover 1 major and 2 minor holidays per year.

• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self‑advocacy and independence.
• Utilize state‑of‑the‑art technology to document skilled services provided.

• Licensed Practical Nurse with a valid CT LPN license.
• One year of LPN experience; home‑care experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.
