Overview
Location:
680 Main Street, Watertown (10326). Shift: Monday–Friday. In‑person position. Requires RN, PT, OT or SLP degree. Primary duty is to analyze Oaiss data, provide change recommendations, and educate staff on Oaiss guidance.

Responsibilities- Review OASIS assessments for accuracy, completeness, and regulatory compliance; provide recommendations to ensure quality.
- Guide staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality.
- Review visit utilization for appropriate patient care; collaborate with clinicians on necessary adjustments; report utilization concerns and financial risks to leadership.
- Ensure timely completion of HCHB workflows and maintain a flexible schedule to support agency needs.
- Collaborate to identify documentation trends, support improvement planning, participate in quality initiatives, and help ensure compliance and accurate data submissions across all settings.
- Degree from an accredited program leading to licensure as RN, PT, OT, or SLP (required).
- Bachelor’s degree in nursing; master’s in PT, OT, or SLP preferred.
- Minimum 2 years as certified home health clinical manager or OASIS reviewer.
- Homecare Homebase experience (OASIS Certification and / or Quality/Compliance Certification).
- 3 years as certified home health clinical manager or OASIS reviewer preferred.
