Team lead position overseeing Direct Support Professionals in providing direct support and training to individuals with intellectual, behavioral, and/or physical disabilities.

• Provide support, development, and implementation of person-centered planning.
• Instruct, supervise, and lead a team of direct support professionals in providing person-centered care, assisting in the development of social, leisure, recreational, communication, safety, and other skills.
• Assist persons served in developing social, leisure, recreational, communication, safety, and other skills.
• Plan, prepare, and lead social and educational activities.
• Ensure environments are comfortable, productive, and stimulating.
• Assist with personal hygiene needs and feeding.
• Plan, prepare, and engage in a variety of community-based activities.
• Utilize agency and/or public transportation, as needed.
• Provide medication management and health-related services.
• Provide thorough written documentation of services, including details of staff interventions and consumer responses as outlined by funding source regulations.
• Develop and maintain positive relationships with families, caregivers, referral sources, funding sources, and other providers.
• Maintain records of persons receiving services according to agency policies and procedures and regulatory requirements.
• Report incidents and other safety issues to the supervisor.

• High School diploma or equivalent.
• Mid-level position requiring one year of experience working with people who have disabilities, preferred.
