Overview

The Judge Group is hiring an Advanced Practice Clinician – Nurse Practitioner / Physician Assistant - for a Senior Community Care (SCC) program in the Waterford area. Within SCC, you will provide care to our highest-risk health plan members while working as part of an interdisciplinary team. This role offers true work-life balance with flexible scheduling, no on-call, no weekends, and no holidays required.

• Conduct comprehensive assessments of patients
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate documentation including ICD-10 coding
• Collaborate with interdisciplinary care teams
• Lead advanced illness and care planning conversations
• Provide counseling and education to patients and caregivers

• Active and unrestricted license
• Education:
  • NP:
    Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • PA:
    Graduate of an accredited Physician Assistant degree program
• Certification:
  • NP:
    Board certified through AANP or ANCC (Family NP, Adult NP, Gerontology NP, or Adult-Gerontology Acute Care NP)
  • PA:
    Board certified by NCCPA
• Current, active DEA licensure/prescriptive authority
