WORK INDEPENDENTLY
· 1-to-1 IN-HOME PATIENT CARE

RESPONSIBILITIES:

Perform home visits for patients in a home care setting.

Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition.

Teach, supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition.

QUALIFICATIONS:

Current license as a Licensed Practical Nurse in Connecticut.

Valid driver’s license.

Reliable transportation and willing to travel in New London County.

Strong communication and interpersonal skills.

Home health experience including Medicare preferred.
