Position: Permanent Supportive Housing Case Manager

Overview

The Permanent Supportive Housing Case Manager will play a crucial role in the State's Permanent Supportive Housing Assistance Medicaid Program. Responsibilities will include assessment and eligibility determination, service authorization, and comprehensive care coordination. This position will be part of a small, dynamic team dedicated to collaborating with community-based service providers to address homelessness in Vermont.

Primary responsibilities include, but are not limited to:

  • Eligibility and Assessment:
    Determine eligibility for program services; conduct whole-person assessments of need, collaborate with participants to co-create person-centered care plans, and conduct annual reviews to assess progress.
  • Medicaid Support:
    Assist with accessing and renewals for Medicaid.
  • Service Coordination:
    Coordinate services with local contracted providers, ensuring that all plans and services align with participants' needs and priorities. Work collaboratively and behind the scenes with providers to deliver effective support.
  • Team-Based Care Coordination:
    Refer to and collaborate with a range of providers including, but not limited to, Designated Agencies, Preferred Providers, Primary Care Physicians, and the VT Chronic Care Initiative, utilizing the Team-Based Care model.
  • Quality Assurance:
    Engage in quality assurance and management activities, including site visits, case reviews, and other monitoring activities to ensure program adherence.

You Bring:

  • A strong commitment to housing first principles, trauma-informed care, and strengths-based relationships.
  • Proficiency in recognizing and addressing health needs including mental health, substance use, complex physical health concerns, delays in cognitive development, and activities of daily living assistance.
  • Familiarity with housing and homeless systems of care, evidence-based and emerging practices in human services and housing, and federal rules and regulations (HUD/Medicaid).
  • Excellent communication skills and the ability to establish and maintain effective working relationships.
  • Keen attention to detail and the ability to thrive in a fast-paced environment.
  • Advanced proficiency with technology and the ability to quickly learn new platforms and databases.
  • Ability to perform job duties within the framework of the four key practices of the Agency of Human Services: customer service, holistic service, strengths-based relationships and results orientation.

  • Competitive salary range: $30.92/per hour - $48.48/per hour.

Although the full salary range for this position is provided, new hires typically start at the salary range minimum and move through the pay steps over time.
