Basic Function
Job Coaching & Onsite Support – assists individuals with disabilities in learning job tasks, developing workplace skills, and successfully integrating into the work environment to promote long‑term job success.
DescriptionPer Diem positions.
$19.00 per hour / Hours vary.
ResponsibilitiesDevelop and adhere to identified C‑CARES plans and relate them to employment opportunities. Implement the plan designed by the C‑CARES Director of Performance‑Based Program for the structured exploration and evaluation of work. Provide onsite job coaching, onsite support, and permanent employment options for consumers as assigned. Develop working relationships with employers to foster job coaching and onsite support. Job coaching services: teach job tasks using adaptive methods, support the development of soft skills (communication, teamwork, problem‑solving, etc.),
and foster independence and self‑advocacy. Onsite support: assist individuals in navigating workplace routines and expectations, provide hands‑on guidance/modeling during work shifts to ensure task completion, and intervene promptly to resolve workplace challenges or misunderstandings. Engage in consumer activities identified through an official CRP (Community Rehabilitation Provider) referral form from the C‑CARES Director of Performance‑Based Program. Prepare reports and conduct other related services as required.
Other duties as assigned. Provide services on a flexible work basis that may include evenings or weekends. Valid CT driver’s licence and working automobile that is insured.
Education:
High school graduate with college experience in the area(s) of education, human services, psychology, or other related area (must go through internal training program).
Knowledge with respect to working with adults or youth with physical/developmental disabilities.
