Overview
Your role as an Unsheltered Outreach Caseworker:
The Unsheltered Outreach Caseworker oversees a variety of cases through different venues on a daily basis in Waterbury, CT and is able to provide the unsheltered seeking assistance the necessary tools to accomplish their goals. They will maintain a schedule including but not limited to hours of actual street outreach, use of equipment and supplies as well as a schedule of external support providers that may able to further the individuals’ attempts to end their homelessness.
Also will work within the program’s model as well as protocols established by the Western CT Mental Health Network and the Coordinated Access Network with whom we jointly run the outreach services establishing open communication, support and accountability among collaborating agencies.

- High school diploma or GED required
- Bilingual in Spanish and English
- Two years in a human service organization preferred
- Two years’ experience working with adults with severe and prolonged mental illness, addiction/recovery or unsheltered populations
- Individuals with lived experience encouraged to apply
- Bachelor’s degree from an accredited educational institution preferred
- Familiarity with PATH program funding
- Reliable, registered & insured vehicle to transport clients and approved driving record check
- Approved criminal background check (CORI)

- Full time:
Monday-Friday 8:30am-4:30pm
