LPN Case Manager - Marjorie Doyle Rockwell Center in Cohoes, NY

We have an opportunity for a LPN to further their nursing skills as a leader and develop strong bonds with their residents and coworkers! What you will do:

Eddy Memory Care at Marjorie Doyle Rockwell Center is a memory care community complete with 52 private and semi-private rooms providing comfortable, secure living in an environment designed for safety and independence. Programs serve individuals with early to mid-stage memory impairment — those with difficulties that require watchfulness and a supportive environment, but who do not need nursing home care. The center is thoughtfully designed and services are tailored for individuals with memory loss, Alzheimer’s disease and other related dementias.

Responsibilities:
  • The LPN Case Manager will help coordinate care for residents who are functionally, physically, and/or socially impaired and need 24-hour oversight
  • Develop relationships and create open lines of communication with resident family members
  • Adhere to DOH standards for Case Management charting and documentation
  • Schedule and facilitate meetings with resident family members

    Assess prospective residents to determine their eligibility for admission at an assisted living level of care
  • Understand and be empathetic to the needs of the geriatric population and always be polite and responsive to residents and their family members
  • Work flexible hours and provide on-call coverage on a rotating basis
What you will need:
  • Valid NYS LPN license
  • 5+ years of experience as an LPN
  • Experience in a case management or care coordinating role is preferred
  • Experience working in memory care as an LPN y desired
  • Maintain the highest level of customer service while completing day to day duties and responsibilities
  • Effective written and verbal communication skills is a must
  • Self-motivated and organized
  • Ability to engage within a team setting

    Passion to learn, grow and develop in the nursing field

Pay range: $26.00-$32.10

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly
