Job Description

The Registered Nurse, RN, is a member of the interdisciplinary team who works together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN is responsible for care coordination, continuous assessment of patients and families, and implementing the interdisciplinary plan of care.

Primary Functions

As part of the team, the RN provides comprehensive assessments, supervises delegated nursing tasks, educates patients and families, participates in team meetings, and ensures quality and compliance standards are met. The RN also participates in quality improvement initiatives and professional development activities.

Responsibilities
  • Case management and supervision of nursing staff and volunteers
  • Following VITAS policies and practicing within the scope of nursing licensure
  • Performing comprehensive assessments and developing care plans
  • Providing patient and family education, including pain management
  • Participating in team meetings and communication
  • Participating in on-call rotations and providing care during non-business hours
  • Delegating tasks appropriately and ensuring safety and compliance
  • Participating in quality improvement and professional development
  • Maintaining documentation and adhering to infection control practices
Minimum Requirements
  • Current RN license in the state of practice
  • Minimum of two years nursing experience in hospice, home health, or community health in the last five years
  • Reliable transportation and available telephone
Physical Demands

Must be able to assist in lifting patients, stand or walk for extended periods, and perform other physical tasks as required. Reasonable accommodations may be made for individuals with disabilities.

Qualifications
  • Licensed to practice nursing in the state of employment
  • Experience in hospice, home health, or community health preferred
