The Case Manger is an integral part of the My House team and is responsible for completing intake, create self-driven goals and connection to needed services that facilitate successful outcomes for our youth. Case managers work closely with Peer Navigators, Outreach, Trafficking, the 9 Star teams and other community supports in breaking down stigma, removing barriers and building supports. Engage in traditional and recovery-oriented peer support tasks following the client’s case plan, including but not limited to, the coordination of related services such as, job training, medical needs, housing, budgeting, skill building, education, recovery and social supports.
Responsible for assigned housing units.
- Conduct a needs assessment, safety assessment, intakes into services and case plans.
- Coordinate services for homeless clients, including referrals for mental health, medical, dental and other ancillary services utilizing partner providers.
- Work closely with Nine Star for a hand up system that trains and empowers homeless youth.
- Coordinate services with Public Health and other medical providers that services for reproductive health, screening for STD/HIV and HEP-C and pregnancy, and birth control.
- Work with family members, the court system, social service agencies, hospitals, and other service providers to offer wraparound services for homeless youth.
- Conduct case staffing’s, provide feedback during case management and staff meetings.
- Spend time in schools and partner organizations working to build relationships that facilitate wraparound services for homeless youth.
- Communicate effectively and be part of a team of staff professionals working together to support and encourage homeless youth to set and accomplish goals, establish healthy boundaries and become productive.
- Participate in groups and organizations in the role of advocate for the organization as well as the homeless youth as needed.
- Maintain a professional posture with clients, community members and partners at all times, using appropriate language and positive feedback designed to offer constructive growth that benefits the overall mission.
- Represent MY House in a positive and proactive manner at all times.
- Facilitate the prompt completion of housing applications move-ins and move-outs.
- Enforces house rules and guidelines by providing support, guidance, and oversight that hold youth accountable in the upkeep of the home.
- Lead weekly house meetings, create weekly house chore assignments sheet and complete weekly sanitation walk troughs ensuring houses are keep up to standards of My House.
- To provide emergency on-call on a rotating basis for designated programs and be available to provide in-person coverage and supports when necessary.
- Other duties and tasks as negotiated.
- Associate’s Degree in Social work, Psychology, Human Services, Counseling, or Substance Abuse/Addiction Counseling. Relevant experience may substitute for educational requirement on a year for year basis.
- Two (2) years providing case management services.
- Strong understanding of community resources and referral agencies
- If in recovery must be in remission for two (2) year and working a program of recovery
- CPR/First Aid/AED certified or able to obtain within 90 days of hire
- Continued employment is contingent upon receipt of a satisfactory state and federal background check, approved National Provider Identifier Standard (NPI) status, and state Approval as a Qualified Addictions Professional (QAP) or a Provisional (QAP)
- Must have a valid Alaska Driver's License and insurable under My House vehicle insurance
