- Provide targeted case management: establishing rapport with clients, writing comprehensive treatment plans, counseling and support, direct skill teaching, advocacy and assistance in obtaining/maintaining benefits, budgeting, treatment, medical services, employment, transportation, etc.
- Maintain family and natural supports and other collateral contacts to ensure quality client‑centered care; assess and involve natural supports in the treatment process, network with homeless service providers, liaise with landlords, and provide overall case coordination.
- Assist in understanding federal/state guidelines for rental subsidies, tenant rights, housing law, and conduct outcome surveys as required.
- Maintain documentation in compliance with agency and program standards.
- Participate in staff meetings, supervision, trainings and other meetings as assigned.
- Perform additional duties as required.
- Education:
Bachelor’s Degree in human services or equivalent education and experience. - Experience:
2 years working with the dually diagnosed (mentally ill, substance abuse). - Licensure/Certification:
Valid driver’s license. CPR/First‑Aid certification within 6 months of hire desired.

$18.00/hr minimum, and up commensurate upon experience. Actual rates are determined at the time of offer and are based on relevant experience and internal equity policy.
