Case Manager – Senior & Independent Living Services

At Rural Resources Community Action, we are looking for a dedicated Case Manager to support individuals and families through our Senior & Independent Living Services program. This full‑time role requires a compassionate, proactive professional who can navigate community resources and advocate for clients in a dynamic environment.

Responsibilities
  • Provide comprehensive case‑management services for clients in the Senior & Independent Living Services program.
  • Assess client needs, develop and monitor service plans, and ensure timely implementation.
  • Make appropriate referrals and coordinate with external service providers.
  • Advocate on behalf of clients during interactions with community agencies.
  • Execute program standards and facilitate the delivery of related services.
  • Maintain knowledge of community resources, laws, and regulations relevant to the area of service.
