The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The RN coordinates the total plan of care with other healthcare professionals involved and helps maintain continuity of care by planning and exchanging information with the physician, agency staff, patient, family, and community resources.

All services are provided within a Point of Care setting.

• Provides clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
• Conducts initial and/or comprehensive nursing evaluations, determines the primary focus of care, develops the plan of care within state-specific guidelines, and ensures accurate, complete, and timely documentation.
• Evaluates and re-evaluates the patient's nursing needs regularly, at least every 60 days or as condition changes.
• Performs comprehensive assessments at designated times and develops the appropriate plan of care in collaboration with physician orders.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by the payer source.
• Initiates, develops, implements, and revises the plan of care collaboratively with physicians and other healthcare professionals.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit
