Description
We are currently seeking a dedicated and experienced Nurse Practitioner (NP) specializing in Family Medicine. As an NP in Family Medicine, you will play a crucial role in providing comprehensive healthcare services to patients of all ages within a family practice setting. This position is vital to our organization as it directly impacts the well-being and health outcomes of our patients.
You will be involved in a variety of projects aimed at promoting preventive care, managing chronic conditions, and fostering patient education.
- Conduct thorough assessments of patients health status and develop appropriate care plans.
- Diagnose and treat common acute illnesses and injuries.
- Manage chronic health conditions such as diabetes, hypertension, and asthma.
- Order and interpret diagnostic tests as needed.
- Prescribe medications and other treatments following evidence-based guidelines.
- Provide patient education on disease prevention, healthy lifestyle choices, and treatment options.
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate and up-to-date medical records.
- Master s degree in Nursing with a specialization in Family Medicine.
- Active Nurse Practitioner license in the state of practice.
- Certification as a Family Nurse Practitioner (FNP).
- Demonstrated experience working in a family practice or primary care setting.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively in a fast-paced environment.
- Proficiency in electronic health record (EHR) systems.
- Schedule:
4x 10-hour shifts per week - No call requirement
- Openings available at multiple locations
- Fully-established patient panels
- 30 minutes per new patient, 15 minutes per follow-up
- Must be board-certified or board-eligible
