JOB SUMMARY
Responsible for directing the development, implementation, supervision and ongoing evaluation of the hospital activities program. Oversees the completion of the activity's component of the comprehensive assessment and the development, implementation and evaluation of the comprehensive care plan goals for each non skilled swing bed resident. Directing the activity program includes scheduling of activities, both individual and groups, implementing and/or delegating the implementation of the programs, monitoring and evaluating the response to the programs to determine if the activities meet the assessed needs of the resident, and making revisions as necessary.
ESSENTIAL DUTIES AND RESPONSIBILITES
- Provides direction for recreational, social and spiritual activities for the facility patients and residents. Interviews the residents, completes section on Minimum Data Sets, documents, maintains quarterly charting timely and at times assists with activities.
- Ensures the comprehensive care plan goals are individualized to match the skills, abilities and interests/preferences of each patient.
- Monitors and evaluates all programs offered to the residents. Includes Residential Care Manager and resident family members in care planning for therapeutic activities. Makes revisions as needed.
- Fills out and completes the Resident Profile and Activities Program Information Sheet within 48 hours of admission.
- Completes the Activities Assessment for each Skilled Swing Bed patient.
- Maintains programs designed to make life more meaningful, to stimulate and support physical and mental capabilities to the fullest extent and to enable the resident to maintain the highest attainable social, physical and emotional functioning.
- Reevaluates the care plan and the specific activity plan quarterly and documents findings in the interdisciplinary progress notes.
- Assists with bus and out of facility outing activities and gets consents from residents or relatives.
- Assists with activities when needed. Makes room visits, holds conversations with residents, prays with them if deemed appropriate, reassures them and gives moral support where necessary.
- Notifies appropriate licensed staff when patient complains of pain.
- Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.
- Keeps current list of new admissions. Is responsible to ensure that the Provider agrees with the activity plan during interdisciplinary meetings, so that the plan does not conflict with the treatment plan.
- Maintains a written planned schedule of social and other purposeful independent or group activities.
- Advocates to CNO the need for additional supplies or equipment to make the activity program successful.
- Plans and conducts in-service training for the nursing staff at least annually.
- Coordinates the activity schedule with other resident services.
- Maintains a current list of residents who are unable physically to participate in activities.
- Develops and implements activities for residents unable to leave their rooms.
- Posts the activity schedule with other resident services in the activity/dining room
- Develops and maintains contact with community agencies and organizations to promote events and volunteer opportunities for the program
- Assures that progress notes specific to the resident's activity plan are recorded at least quarterly, and more frequently if needed, in the resident's or patient's electronic health record.
- Is able to assist residents with activities; is compassionate and understanding and is able to listen and converse with older people.
- Monitors staff compliance of activity documentation, educates staff as needed.
- Covers the scheduled role of nurse assistant as needed in order to maintain staffing levels determined by Management.
- Perform other duties as assigned by Management.
- Actively checks and responds to emails throughout the day.
- Completes all mandatory education within given timeframe.
- Maintains regulatory requirements.
- Reports to work on time and as scheduled.
- Consults with CNO prior to working overtime.
- Wears identification while on duty; uses computerized punch time system correctly.
- Represents the organization in a positive and professional manner in the community.
- Attends annual review and department in-services as scheduled.
- Attends at least 75% of staff meetings; reviews all monthly staff meeting minutes.
- Actively participates in the department's quality improvement (QI) activities.
- Complies with all organizational policies regarding ethical business practices and the employee handbook.
- Communicates the mission, vision and values of the organization as well as the focus of the department.
