Physical Therapist – Hybrid Position (SNF) – 2 to 3 days in Person a week

We’re seeking a motivated and compassionate Physical Therapist to join our team in a hybrid role serving residents in a Skilled Nursing Facility (SNF) setting. This flexible opportunity is ideal for clinicians looking to combine the benefits of in‑person patient care with the convenience of telehealth practice.

Position Details
  • Hybrid schedule: 2–3 days on-site; remaining days remote via telehealth
  • Patient population:
    Skilled Nursing Facility residents, short‑term rehab and long‑term care
  • Employment type:

    Full‑time or part‑time considered
  • Start date:

    Flexible
What You’ll Do
  • Evaluate and treat patients with a variety of physical impairments in alignment with individualized care plans
  • Deliver high‑quality therapy services both in‑person and via telehealth platforms
  • Collaborate with an interdisciplinary team to support patient outcomes and functional independence
  • Document services in compliance with regulatory and payer requirements
Qualifications
  • Current Oregon Physical Therapist (PT) license (or eligibility)
  • Prior experience in SNF or geriatrics preferred
  • Comfortable with or willing to learn telehealth delivery
  • Strong communication and clinical reasoning skills
