Case Manager - Short Term Family HousingHillcrest Children & Family Center is a non-profit organization dedicated to the provision of comprehensive, evidence-based mental health services in Washington, DC. We are committed to fostering community awareness, eliminating systemic barriers, and delivering compassionate, high-quality care to individuals navigating housing challenges.
Case Managers serve as the primary point of contact for families and carry responsibility for housing stabilization, service coordination, and documentation. Each Case Manager supports families, many of whom face multiple barriers including housing instability, behavioral health needs, domestic violence histories, and limited income. Staff must develop individualized plans, coordinate benefits and services, support housing placement, and respond to crises.
Essential Duties and Responsibilities
- Staff must develop individualized plans, coordinate benefits and services, support housing placement, and respond to crises.
- Act as a liaison between the family and potential landlords
- Work with families to create a realistic household budget based on their current income or pending benefits
- Maintain strong relationships with housing authorities, and community partners
- Ensures accurate and timely data entry, reporting, and compliance documentation
- Rapid problem-solving in crisis situations
- Ability to work outside of standard business hours, through on-call rotation, to assist or manage participant crisis
- Ability to manage high-stress caseloads and maintain continuity of care for families during critical stabilization periods.
Qualifications:
- Bachelor’s degree in psychology, human services field, or equivalent
- 1-3 years of experience in behavioral health, or working in a housing program
- Reliable transportation for community-based visits is required
- Working knowledge and experience in housing systems, family homelessness, trauma informed supervision, and government contract management. Consumer advocacy or self-help group preferred.
- Capable of ensuring program stability, regulatory compliance, and strong outcomes in a highly demanding environment.
- Ability to set appropriate boundaries for best practice.
- A sound understanding of mental health, housing requirements, and behavioral health programs
- Demonstrates proactive approaches to problem-solving with strong decision-making capability.
- Demonstrated ability to achieve high performance goals and meet deadlines in a fast-paced environment.
- Proficient in the use of Microsoft Office Suite (Word, Excel, Outlook, etc.)
