Community of Hope is seeking a full-time Housing Stability Case Manager for our Permanent Supportive Housing;
Site Based Program. In this role, you’ll help families and/or individuals secure and maintain stable housing, increase self-sufficiency through income, benefits, and education, and strengthen overall family stability. This position is located at Mississippi Ave in Southeast, DC.

What You’ll Do
  • Welcomes individual adults and families to the program, reviews and explains program rules and client rights and responsibilities, and provides each client with a copy of the Program Rules.
  • Works with clients to develop individual goal plans that are measurable with action steps; updates goal plans according to program contract guidelines and as needed, signed by client and case manager.
  • Networks and coordinates with community resources, makes referrals as appropriate, and follows up to confirm and track clients’ participation in housing, employment, medical care, substance abuse treatment, mental health treatment, training, etc.
  • Assists clients with housing search in collaboration with Housing Specialist; accompanies clients to view units, advocates for clients with landlords, and assists with applications and paperwork. If COH provides financial assistance for application fees, accompanies clients to render payment.
  • Conducts scheduled and unscheduled home visits and provides tenant education in accordance with program contract guidelines, as needed.
  • Maintains regular communication with all clients on caseload and documents progressive engagement efforts with difficult-to-engage clients.
  • Works within a housing-first framework, ensuring services are strengths-based and client-centered.
  • Performs other duties as assigned.
Requirements

Must-Haves

  • Bachelor’s degree in social work or related field required or 4 years social service experience.
  • Minimum one year work experience in social service or related field required with degree. Minimum four years work experience in social service field if no degree.
  • Strong interpersonal, conflict-resolution, and organizational skills required.
  • Ability to work independently and as part of a team required.
  • A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.

Nice-to-Haves

  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly preferred.
