Job Description - RN Clinical Program Coordinator - ENT Clinic (250001AP)
The Clinical Program Coordinator will complete hospital and nursing orientation y will coordinate nursing care and services for patients and families in specific healthcare specialties through telephonic, electronic, in person, and mail interactions. They will consult with physicians and other healthcare providers to resolve health concerns.
All nursing practice is based on the legal scope of practice, national and specialty nursing standards, CNMC policies, and applicable laws. The Professional Model of Care requires registered nurses to be responsible and accountable for their practice. CNMC supports the development of RN relationships within the community, focusing on health and wellbeing. The Coordinator leads the patient care delivery team for a specific patient population, ensuring continuity of care through assessment, planning, implementation, and evaluation.
Qualifications- Minimum Education: Bachelor's Degree (Required)
- Minimum
Work Experience:
5 years of nursing experience, including at least 1 year recent pediatric experience in a comparable setting, and 1 year of nursing leadership experience (e.g., charge nurse, educator, preceptor).
- Participate in/support Shared Nursing Leadership and contribute to nursing excellence and Magnet designation.
- Engage in committees, professional organizations, and community activities.
- Support performance improvement processes and report on Nurse Sensitive Indicators (NSIs).
- Integrate evidence-based practice and assist physicians to maintain cost-effective, high-quality outcomes.
- Speak up on unsafe behaviors, verify information, and communicate clearly and timely.
- Promote safety for all, practicing self-check behaviors like Stop, Think, Act, Review.
- Collaborate with patients, families, and healthcare team members; involve them in decision-making.
- Maintain authority and accountability for clinical care, develop goals, and document care thoroughly.
- Use the nursing process to assess, analyze, and address patient concerns, providing health education and cultural competency.
- Coordinate care across systems and develop policies to guide practice.
- Maintain medical equipment and supplies, ensure regulatory compliance, and stay updated on health informatics systems.
- Apply operational and financial knowledge to ensure quality, cost-effective care.
- Enhance knowledge and skills in clinical, legal, regulatory, and cultural aspects of care.
- Foster collaboration and a high-performance team environment.
- Facilitate interdisciplinary rounds and conferences.
- Manage scheduling, coverage, and patient flow efficiently.
- Collaborate with staff and communicate issues appropriately.
- Participate in peer reviews.
- Anticipate and respond to customer needs, follow up until met.
- Demonstrate respect, collaborate, and value others’ ideas.
- Contribute to a positive environment, demonstrate flexibility, and identify process improvements.
