Community of Hope is seeking a Clinical Nurse Manager to provide comprehensive, high-quality healthcare services to individuals and families who need it most. You'll work closely with our nurses, medical assistants and other care team members, providing team-based clinical care to patients while ensuring that COH clinical protocols are followed. This position is located at our Conway Health and Resource Center in SW, Washington, DC.

What You’ll Do
  • Ensures high quality of care, smooth patient flow and team coordination for patient visits while following the guidelines of the Patient Centered Medical Home model and/or Birth Center model.
  • Direct supervision of Medical Assistant I and Medical Assistant I. Assists with coordinating Nurse and Medical Assistant Interns, ensuring proper training of both.
  • Serves as a liaison between nurses, providers and other office staff.
  • Prepares patients for examination and treatment including, but not limited to, taking the patient’s history, obtaining accurate vital signs, and verifying the chief complaint.
  • Assists with clinical procedures done by the providers in the office as needed.
  • Assists with Chronic Care Management, by providing care and education to patients, including but not limited to, conducting Previsit Chart Reviews as necessary.
  • Maintains a pap log to ensure that proper follow up from abnormal paps is accomplished within the designated time.
  • Monitors the outstanding lab report in eCW to ensure that results are received in a timely manner and that patients return to the clinic should a lab not be completed properly.
  • Maintains a Newborn Metabolic Screen log to ensure that results are obtained in a timely manner for all Newborn patients.
  • Monitors the functioning of the Vaccine for Children program at the clinic according to CDC and DC regulations.
  • Meets with supervises regularly and provides real-time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns promptly as needed.
  • Reviews the performance of team members in a timely manner, including completing 90 day and annual performance evaluations per their due dates, documenting any issues or changes, providing recommendations for professional development and/or training opportunities, and recognizing staff when appropriate.
  • Implements controlled substance regulations as necessary (For FHBC and Conway only).
Must-Haves
  • A current, unencumbered DC Registered Nurse license.
  • Valid BLS Certificate.
  • One year of supervisory experience.
  • One year working in an ambulatory, primary care or med/surg setting.
  • Knowledge of chronic disease processes and health maintenance.
  • Ability to work with computers and electronic health records.
  • Strong verbal and written communication skills.
  • Maintain current certification as required by law, including, but not limited to healthcare provider CPR or NRP depending upon clinic location.
  • Proof of vaccinations. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
Nice-to-Haves
  • Bilingual in Spanish and/or Amharic.
  • Prior care management and patient education experience.
  • Experience working with patients with substance use disorders and behavioral health concerns.
