Role Summary Provide one-on-one skilled nursing care to patients in their homes.
Key Duties
- In-home nursing assessments
- Wound care, injections, catheter care, meds
- Monitor condition & report changes
- Patient/caregiver education
- EHR documentation
- Team collaboration & safety compliance
Requirements
- Active LPN/LVN license
- 2+ years nursing experience (home health preferred)
- Strong communication & assessment skills
- EHR proficiency
- Valid driver’s license & reliable transportation
