Position: Case Manager (Independent Contractor)

Overview

SOME (So Others Might Eat) provides material aid and comfort to our vulnerable neighbors in the District of Columbia, helping them to break the cycle of poverty and homelessness through programs and services that save lives, improve lives, and help to transform the lives of individuals and families, their communities, and the systems and structures that affect them. We meet immediate needs with food, clothing, and healthcare, and offer the tools one needs to live with hope, dignity, and greater independence.

Compensation

Position Description

The Contractor shall provide case management and supportive services to assist participants in identifying and addressing barriers to training and employment, while promoting stability, engagement, and successful program outcomes.

Qualifications

Required: Bachelor s degree in Social Work or related profession, 2 yrs in case management, counseling, community engagement, development, education outreach, or faith-based organizing.

Preferred License/Certification: Social Work License with LGSW or LICSW preferred

Schedule

Schedule: 4 hours a day / 2 x week. To not exceed 8 hours a week.

Responsibilities
  • Intake & Service Planning: Conduct comprehensive intake assessments and develop individualized service plans aligned with participants  training and employment goals.
  • Ongoing Support: Provide short-term counseling, coaching, and crisis intervention as needed to support participant stability and engagement.
  • Barrier Resolution: Assist participants with addressing barriers, including housing instability, food insecurity, transportation, childcare, financial challenges, non-clinical mental health concerns, substance use recovery (via referrals), and legal or justice-related issues.
  • Resource Coordination & Advocacy: Maintain knowledge of community resources and public benefits; facilitate referrals to internal and external partners; and advocate on behalf of participants when appropriate.
  • Documentation & Data Tracking: Maintain accurate, timely, and confidential case notes; track services, referrals, and outcomes in required databases; and support reporting for funders, audits, and program evaluation.
  • Compliance: Ensure adherence to confidentiality, data privacy, and ethical standards.
Deliverables
  • Completed intake assessments and individualized service plans
  • Ongoing participant support and referral coordination
  • Accurate case documentation and required data entry
  • Contributions to required reports and evaluations
Service Delivery

The Contractor shall perform services primarily on-site, as the role requires in-person, one-on-one trainee or counseling meetings, which are essential to effective program delivery. Administrative tasks, including notes management and data entry, may be completed on-site or off-site, provided all required in-person services are fulfilled.

Physical Demands

Must be able to lift up to 20 pounds. Requires looking at a computer screen for several hours a day. May be required to sit for long periods.
