• Collaborates with healthcare teams to develop and coordinate comprehensive, patient-centered care and discharge plans.
• Conducts psychosocial assessments and provides emotional and behavioral support for patients and families.
• Connects patients with community resources and ensures timely referrals based on identified needs.
• Maintains accurate documentation and communicates effectively with caregivers, case managers, and external agencies.
• Adheres to regulations and safety standards while actively participating in quality improvement, education, and outreach efforts.

• Master’s degree in social work from a school accredited by the Council on Social Work (Required)
• 1-2 years
  - Experience in social work in a hospital setting (Preferred)
• 1-2 years
  - Experience in care/case management (Preferred)

• LMSW
  - Licensed Master Social Worker
  - State Licensure in the District of Columbia or the State of Maryland, depending on work location or
• LGSW
  - Licensed Graduate Social Worker in the District of Columbia or the State of Maryland, depending on work location
• CCM
  - Certified Case Manager (Preferred)

Hiring Range: USD $61,838.00 - USD $ /Yr.
