IDD Case Manager

We are seeking a skilled and compassionate IDD Case Manager to join our team and support individuals with intellectual and developmental disabilities in residential settings. This role is ideal for a dedicated professional with hands‑on experience who is committed to advocating for individuals' well‑being, independence, and quality of life.

Location: Washington, DC (Must be a DC resident)
Schedule: Monday‑Friday, 8:30 AM - 5:00 PM
Work Model: Remote with required weekly site visits throughout Washington, DC

• Conduct comprehensive assessments to evaluate residents' needs, strengths, and areas for support
• Develop, implement, and monitor individualized service plans in collaboration with interdisciplinary teams
• Interview residents and family members to ensure person‑centered care and inclusion of their perspectives
• Maintain accurate and timely documentation of resident progress and changes in needs
• Coordinate with external agencies and funding sources, including Medicaid and Medicare, to ensure proper documentation and compliance
• Advocate for residents' rights, community inclusion, and access to appropriate services
• Provide guidance and training to staff on best practices for supporting individuals with developmental disabilities
• Stay current on industry standards, best practices, and advancements in developmental disability services
