Job Description

Care Support Specialist ( ) – Works as a member of the Care Management Team assisting Care Managers with the coordination and arrangement of health care services (medical and behavioral) and community-based services/social needs for complex, condition‑specific, and/or non‑compliant enrollees in accordance with established care coordination plans. Works with the Care Manager to enrollees and their families.

Qualifications
  • Minimum Education
    :
    High School Diploma or GED (Required). Associate's Degree (Preferred).
  • Minimum Work Experience
    : 1 year experience with the target population in the health and human service field (Required).
  • Required Skills/Knowledge
    :
    Proficient with computer systems and Microsoft Office Suite; effective communication skills; must have a valid driver’s license, a vehicle and/or accessibility to a vehicle.
  • Required Licenses and Certifications
    :
    Driver's License upon hire (Required).
Job Functions
  • Receives direction from the Supervisory Care Manager; reviews/prioritizes tasks received by the Care Manager.
  • Assists with coordinating care and services documented in the care plan for complex, condition‑specific, and/or non‑compliant enrollees.
  • Partners with families to achieve identified goals in the Care Plan.
  • Provides education information, as directed, to the enrollee and their family.
  • Conducts in‑person visits with enrollees, including in the hospital during and following inpatient and emergency department discharge to obtain and/or share pertinent information.
  • Assists enrollees/families with scheduling and completing EPSDT, mental health, medical, and other specialty appointments as needed.
  • Assists enrollees/families with locating community support services and makes referrals to community‑based and other services.
  • Participates in home visits to determine the needs of enrollees and/or gather pertinent information related to the enrollee.
  • Contacts resources—schools, clinics, physician offices and other service providers—to gather information necessary for timely delivery of services.
  • Documents interaction with enrollees, caregivers, providers, and others in the HSCSN IT system within 24 hours of completion.
  • Participates in team meetings, trainings, and other required activities.
  • Assists Care Managers in reaching enrollees that are unable to contact or engage in the community, including conducting visits in the community to assist in locating and identifying correct addresses.
  • Clearly communicates verbally and in writing.
