Community Outreach Coordinator; Healthcare

Overview

Elaine Ellis Center of Health, a federally qualified health center in Washington, DC, is looking for a Community Outreach Coordinator to provide healthcare outreach services in the DC and Maryland area.

• Conducts all programmatic outreach activities including school outreach and senior centers.
• Provides program information to community collaborators.
• Links outreached persons to program counseling and care services.
• Collaborates with local community-based organizations.
• Assists Program Coordinator, Case Manager and Outreach team in establishing community linkages and designated outreach venues to expand client base.
• Collects and reports data around member enrollment/activities and contractual requirements.
• Completes all necessary paperwork required by HRSA and enters data into appropriate data collection systems.
• Involved in program evaluation activities including assessing Social Determinants of Health (SDOH).
• Collaborates with other departments/units to develop mutually supportive program-wide services, supports, and opportunities for program members toward optimizing outreach opportunities.
• Participates in agency-wide outreach events.
• Other duties which may be assigned from time to time.

• Bilingual (Spanish-English) with a positive upbeat attitude and keen attention to detail.
