Community Support Worker

Responsibilities
  • Under the direction of a clinical manager, provide support to consumers and their families or caregivers with mental health and substance abuse related issues.
  • Facilitate individual mental health intervention and activities that are strength-based and focused on promoting recovery, symptom reduction, and increasing coping skills to achieve the highest level of functioning in the community.
  • Provide medically necessary services to the consumer in accordance with the treatment plan designed to meet mental health, educational, vocational, residential, financial, social and other needs of the consumer.
  • Follow Mental Health Rehabilitation Services (MHRS) guidelines.
  • Ensure treatment plan authorizations/reauthorizations, CALOCUS/LOCUS and updated diagnostic assessments are current and do not expire.
  • Serve as an integral member of the SLS team to ensure continuity of care and support services 24/7 as necessary.
  • Provide structured face-to-face sessions with consumers at least once per month, or as needed based on the treatment plans.
  • Provide services in home, school, office, and community settings with the majority of mental health services rendered in the community.
  • Complete documentation within 24 hours of service or report to supervisor via phone.
  • Participate in mandatory training and assist with facilitation of staff meetings and trainings.
  • Collaborate with and provide ongoing updates to the clinical team and psychiatrist, as needed.
  • Identify and continually collaborate with community agencies, providers, and natural supports; advise team to make referrals as needed.
  • Interact with clients, coworkers, management, and other professionals in a nonjudgmental manner; communicate objectively, thoroughly, accurately and concisely.
  • Demonstrate understanding and acceptance of the organizational mission to provide quality services.
  • Participate in efforts to enhance service provision to consumers; build cohesiveness and positive rapport with other units and community agencies.
  • Demonstrate professional work ethics, dependability, conscientiousness, and timeliness.
  • Other duties as assigned.
Education & Experience

Bachelor's degree or at least 2 years of related work experience.

Physical Demands

While performing the duties of this job, the employee is regularly required to perform physical activities, drive, stand and walk. The employee may be required to restrain a client or physically intervene to resolve an altercation.
