Overview

Social Worker PRN (240003DE) provides social work services to assist families and caregivers with adjustment to health conditions and hospitalization of children/teens. The role supports problem solving and coping, links clients to resources, and manages care coordination and discharge planning through effective communication with payers and the interdisciplinary team.

Responsibilities
  • Provides assessment of psychosocial, environmental, age-specific, community resource availability and financial impact of illness on patients and their family system, need for care, response to treatment and adjustment to care.
  • Refer patients/families to appropriate human service resources and follow up to ascertain need fulfillment, acting as an advocate to facilitate holistic care. Resources include financial, health, educational, relief aid and equipment.
  • Participate in development of the plan of care.
  • Participate in individual case conferences to facilitate coordination of care and understanding of social and emotional needs. Identify social problems and assess family strengths/weaknesses with focus on alleviating the situation.
  • Document evaluation, treatment and response to the plan of care in an accurate, complete and timely manner.
  • Provide counseling and emotional support and/or crisis intervention as per professional standards.
  • Communicate patient/family assessment information and service plan to providers and health care team. Provide comprehensive assessment to referrals.
  • Ensure patient/family goals are considered when formulating the plan of care.
  • Locate appropriate community resources to meet patient and family needs.
  • Serve as liaison/advocate between patient and community resources, keeping the patient’s best interests foremost.
  • Perform initial assessment of patient/family spiritual needs and make referrals as appropriate.
  • Assist with anxiety and stress related to illness, hospitalization, loss, disability, and death by implementing problem solving and supportive care to reduce emotional discomfort and aid adjustment.
  • Interview and assess for potential referral to entitlement programs and coordinate identified billing problems with the Case Manager.
  • Provide terminal care planning and grief counseling to patients, families and staff.
  • Demonstrate knowledge of options available and communicate them in a supportive manner.
  • Coordinate and participate in family meetings.
  • Maintain up-to-date referral information on locality resources and direct clients to appropriate agencies in a timely manner; review inpatients to identify social service needs.
  • Participate in hospital committee meetings and interdisciplinary team meetings to facilitate coordination of care in the hospital’s Social Work Program.
  • Adhere to current professional standards for social workers and Joint Commission standards; implement changes in treatment modalities, policies, procedures, and programs as required.
  • Maintain all appropriate documentation in an accurate, complete and timely manner.
  • Attend court hearings and DHS/administrative review hearings as appropriate.
  • Provide clinical oversight for Social Worker/Social Work student.
  • Schedule clinic and post-discharge follow up appointments.
  • May perform other duties in addition to those outlined in this job description.
Qualifications
  • Minimum Education
    :
    Master’s Degree in Social Work (Required)
  • Minimum Work Experience
    : 5 years of social work experience (Required); 2 years of experience with in-patient neonatal/pediatric ICU, pediatric sub-acute and/or pediatric rehabilitation (Preferred)
  • Required Skills/Knowledge
    :
    Good assessment skills; ability to function independently; superior time management; outstanding oral and written communication; well-developed computer skills; ability to manage multiple priorities and make decisions; knowledge of discharge planning; working knowledge of Medicaid and private insurance procedures; ability to solve complex problems; family/child experience preferred
  • Required Licenses and Certifications
    : LICSW licensure from the District of Columbia (Required)
