Are you a compassionate and experienced LPN looking for a meaningful way to make an impact—while enjoying the flexibility of community-based care and the support of a collaborative team? At VNA of Care New England, we’re seeking Licensed Practical Nurses to join our Hospice team
, delivering personalized, one-on-one care.

What You’ll Do

As a Hospice LPN, you’ll provide hands-on clinical care and support to patients in their homes, while working closely with a multidisciplinary team. You’ll help manage care plans, monitor, and ensure each patient receives the compassionate and skilled care they deserve.

Your Day-to-Day Might Include
  • Providing direct nursing care, including treatments, wound care, and medication administration
  • Monitoring patient conditions and reporting changes to the RN or care team
  • Supporting the development and adjustment of individualized care plans
  • Collaborating with nurses, therapists, and other healthcare professionals
  • Educating patients and caregivers to promote independence and self-care
  • Accurately documenting all care provided and patient updates
  • Participating in team meetings and interdisciplinary coordination

What You’ll Need
  • Valid Rhode Island LPN license (required)
  • Equivalent education required for RI LPN licensure.
  • At least 1 year of progressive clinical experience (home health or acute care preferred)
  • Current CPR certification
  • Valid driver’s license and auto insurance
  • Strong clinical, communication, and documentation skills
