, delivering personalized, one-on-one care.
As a Hospice LPN, you’ll provide hands-on clinical care and support to patients in their homes, while working closely with a multidisciplinary team. You’ll help manage care plans, monitor, and ensure each patient receives the compassionate and skilled care they deserve.
Your Day-to-Day Might Include- Providing direct nursing care, including treatments, wound care, and medication administration
- Monitoring patient conditions and reporting changes to the RN or care team
- Supporting the development and adjustment of individualized care plans
- Collaborating with nurses, therapists, and other healthcare professionals
- Educating patients and caregivers to promote independence and self-care
- Accurately documenting all care provided and patient updates
- Participating in team meetings and interdisciplinary coordination
- A meaningful, patient-focused role in hospice care

- Valid Rhode Island LPN license (required)
- Equivalent education required for RI LPN licensure.
- At least 1 year of progressive clinical experience (home health or acute care preferred)
- Current CPR certification
- Valid driver’s license and auto insurance
- Strong clinical, communication, and documentation skills
