Overview- General Responsibilities:
We provide culturally competent care to patients from birth through late adulthood, utilizing a multidisciplinary team approach. As a registered nurse you are an essential member of the team delivering care in areas such as family medicine, primary care, pediatrics, obstetrical and gynecological care, medication assisted treatment, trans and behavioral health. Responsibilities include, but are not limited to, competent patient assessment, promptly prioritizing triage calls, determining appropriate patient disposition, managing nurse visits, providing direct patient care, and assisting providers as needed.
The registered nurse acts as a resource person for medical assistants and other team members.

- Conduct competent patient assessments and promptly triage calls, determining appropriate disposition.
- Manage nurse visits and serve as a resource for medical assistants and other team members.
- Support various population needs, including trans health and mental health as part of primary care.
- Administer vaccines and other injections as ordered, and perform treatments ordered by providers (e.g., dressing changes, respiratory treatments).
- Review externally ordered labs and diagnostic imaging and notify providers of abnormal results per policy.
- Obtain and document vital signs accurately (weight, blood pressure, BMI, pain, temperature).
- Triage phone calls and evaluate patient needs using telephonic triage protocols.
- Perform nurse visits and provide evidence-based health education at an appropriate level of understanding.
- Support obstetric patients in pre-natal and post-natal care, triage, and education via nursing and/or provider visits, and telephonically (Applicable to Woonsocket and WW only).
- Support pediatric patients through accurate assessment and triage; administer immunizations and provider-ordered treatments.
- Perform medication reconciliation and process prescriptions per workflows; ensure infection control precautions.
- Act as a resource for other staff; notify patients of test results under provider guidance and educate as needed.
- Develop and implement nursing care plans based on patient evaluation; maintain communication with supervisor, providers, and staff.
- Participate in performance improvement activities and respond to codes and rapid responses.
- Utilize standing orders and SOPs; maintain OSHA and HIPAA guidelines; adhere to organizational policies and procedures.
- Utilize EMR alerts effectively and maintain standard precautions; pursue professional continuing education.
- Perform other tests, duties, and special projects as assigned; use RN manuals, workflows, and instructional materials to perform at top of license.
- Required Qualifications:
Current State of Rhode Island Registered Nurse license; ability to communicate effectively and collaborate with patients, providers, and the healthcare team; professionalism, self-starter attitude, leadership, triage, documentation, and teaching skills; CPR certification. - Preferred Qualifications:
Three or more years nursing experience preferred; new RN graduates will be considered; ambulatory health care experience preferred; candidates who do not meet all preferred qualifications are encouraged to apply.
- This role is onsite.
- Moderate physical activity, including walking, standing, stooping, and occasional lifting.
- Potential exposure to blood-borne pathogens, biohazardous materials, and infectious diseases (e.g., TB, COVID-19).
- Manual dexterity required for injections, immunizations, and computer use.
- As scheduled during hours of operation (including weekend and evening hours as assigned) with occasional unscheduled overtime.
- Communication
Skills:
Demonstrate cultural sensitivity and adapt communication to patient demographics; use teach-back, active listening, and written/visual materials. Document clearly in the medical record; maintain patient confidentiality.
