About the role

We are seeking two experienced Adult Social Workers to join the Hospital Discharge Team (Transfer of Care Hub) at Warrington Hospital. This is a fast-paced, high-impact role where you will play a crucial part in supporting safe, timely, and well-coordinated hospital discharges
.

Working closely with health professionals and multidisciplinary partners, you’ll assess social care needs, reduce delayed transfers of care, and ensure individuals leave hospital with the right support in place. This full-time role offers a hybrid working arrangement, with approximately three days per week based on-site.

What you will do

Work collaboratively with health and multidisciplinary teams to coordinate safe and timely hospital discharges
.

Complete strengths-based assessments to identify patients’ social care needs.

Support effective communication between hospital, community, and social care services.

Actively contribute to reducing delayed transfers of care (DTOC).

Ensure care plans promote independence and meet statutory responsibilities.

Record and manage case information accurately, using systems such as Mosaic where required.

Maintain a strong focus on patient wellbeing, dignity, and safeguarding throughout the discharge process.
