Intermediate Care Staff Nurse

A dynamic dedicated nurse is required to complement our current team of staff within our intermediate care bed base. Providing rehabilitation for both stepdown and step up patients, you need to have a good all round skill set to support the needs of these patients.

Working as part of a dedicated integrated health and social care team you will develop a variety of skills. A foundation of basic nursing skills is required.

This post is to work permanent night, however there will be a requirement for the successful candidate to work occasional day shifts for training purposes.

Main duties of the job
  • Ensuring a high standard of care within an intermediate care bed-based facility.
  • Liaises with the Intermediate Care Sister / Charge Nurse to ensure information in relation to patient care / changes to care plan and continuity of care.
  • Provides mentoring, teaching and assessing to support staff.
  • Liaises with all healthcare professionals and other agencies to ensure effective service delivery in the intermediate care facility.
  • Performs comprehensive assessment of patient nursing needs.
  • Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.
  • Maintain accurate records in accordance with NMC.
  • Attend a multi-disciplinary case conference as the named nurse if appropriate.
  • Communicate and collaborate with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care.
  • Establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
  • Competently perform clinical skills related to the post.
  • Be professionally responsible for delegated visits under the supervision of the Intermediate Care Sister / Charge Nurse.
  • Work without direct supervision.

Job responsibilities

Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on 1st April 2026. If your start date is on or after 1st April 2026, you will be employed by the newly formed organisation. If you join Bridgewater prior to this date, your employment will automatically transfer into the new organisation.

Job Purpose:

  • Ensuring a high standard of care with an intermediate care bed-based facility.
  • Liaising with the Intermediate Care Sister / Charge Nurse to ensure information in relation to patient care / changes to care plan and continuity of care.
  • Providing mentoring, teaching and assessing to support staff.
  • Liaising with all healthcare professionals and other agencies to ensure effective service delivery in the intermediate care facility.

Managerial

  • Be responsible for work allocations of support staff.
  • Complete monthly supervisions for support staff.
  • Be responsible for organising and prioritising own caseload.

Leadership

  • Support the development of the nursing team by teaching and assessing members of the team including nursing assistants.
  • Undertake mandatory in-house training and other training that supports the role.
  • Access clinical supervision.
  • Undertake training specific to the post.
  • Teaching of patients / carers and other professionals to support patients to maintain their optimum level of independence.

Clinical

  • Perform comprehensive assessment of patient nursing needs.
  • Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.
