Bridgewater Community NHS Foundation Trust have an exciting opportunity for acaring and ambitious SCPHN school nurse to join our school nursing team and bean integral part of our 3 well established integrated teams within our Warrington 0-19 service. Main duties of the job
  • Be a qualified Specialist Community Public Health Nurse (School Nursing).
  • Provide supportive leadership within the 0-19 team
  • Bean autonomous practitioner who can support the needs of the team and service.
  • Takepart in service development, promote innovation and demonstrate skills inpublic health.
  • Havea flexible and conscientious approach to tasks.
  • Beable to communicate effectively with partner agencies, promoting the work and reputation of the service, working for the best interests of the child, and adopting a think family approach.
  • Asa key professional in the protection and support of children and young people, you will be required to work collaboratively with other agencies to meet their needs.

Job responsibilities
  • The named School Nurse will be responsible for co‑ordination of the School Health Plan with their named schools on an annual basis.
  • Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies
  • Lead on the recommended health assessment and reviews including:
    Reception/Year 1 (age 45) school entry assessment (transition from 05 HCP school entry questionnaire;
    Providing Year 6/7 (age 1012) assessment at transition from primary to secondary school.
  • Supporting mid‑teens reviews, when young people are embarking on the next transition stage as and when required in national and local specifications.
  • Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met.
  • Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4‑19 years of age.
  • To contribute to the delivery of the national immunisation programme for school aged children.
  • Plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Consult, advise and educate children, young people and families to enable them to effectively manage their health needs.
