Nurse Navigator – Hourly position. This Casual, as‑needed role is situated in the Intake Department of the Proton Center, assisting patients through scheduling, orientation, and coordination of multidisciplinary care. Hours are flexible.
Responsibilities- Clinically competent, registered nurse coordinating multidisciplinary care using the nursing process.
- Collaborates with patients, families, and the healthcare team while maintaining ANA standards.
- Assesses and reassesses patient and family care needs, initiating referrals as appropriate.
- Collects and analyzes data for new patients, identifies and coordinates care, and schedules appointments.
- Triage and coordinate new patients for multidisciplinary clinics.
- Assist in creating a plan of care that reflects collaboration and communication.
- Utilizes evidence‑based research to guide practice decisions.
- Awareness of clinical research trials and collaborates with research staff for eligible patients.
- Supports non‑clinical staff with information about prospective patients.
- Provides rationale for clinical decisions based on critical thinking.
- Performs all patient care aspects to optimize safety and reduce errors.
- Identifies learning needs, assesses support systems, coping mechanisms, and barriers; makes referrals.
- Educates patients, families, and caregivers on diagnosis, therapy, and testing, assisting transitions.
- Demonstrates interest in personal development beyond routine educational sessions.
- Current experience within the past five years.
- Completion of an accredited BSN program.
- RN license in Illinois (LIC RN).
- Basic Life Support CPR certification.
- Basic computer knowledge.
- Effective communication in English, both verbal and written.
- Excellent customer service skills.
- Works within Illinois Practice Act and ANA standards.
- Ability to assess interpreter needs and use interpreter services.
- One year of acute or outpatient experience within last four years.
- Case management or patient navigation experience.
- Basic EPIC EMR knowledge.
