The Clinical Coordinator (Post Acute and Population Health), will support staff in coordinating and managing care for Endeavor patients across post-acute and ambulatory settings. This role focuses on optimizing clinical outcomes, resource utilization, and patient care coordination across various care settings and providers. The Clinical Coordinator will assist in ensuring the delivery of high-quality care and service excellence, while contributing to the effective management of clinical resources.
They will collaborate with physicians, clinical leaders, and interdisciplinary teams to ensure seamless care transitions and continuity for patients. Additionally, the Clinical Coordinator will provide support in the operational and administrative functions of care coordination across the post-acute network. This position requires strong leadership skills, clinical expertise, and a commitment to excellence in healthcare.
What you will do:
- Ensures the Delivery of High-Quality Care Management
a) Establishes and maintains care standards that align with community practices and professional guidelines.
b) Collaborate with other departments, including physicians, care coordination teams, and ancillary services, to coordinate patient care and enhance the patient experience.
c) Oversee the accurate and comprehensive documentation of patient assessments, care plans, and interventions within the electronic health record system.
d) Ensure strict adherence to all state and federal regulations, as well as policies and procedures, within the unit.
e) Oversee the delivery of patient-centered care within the unit. Collaborate with healthcare providers to develop and implement care plans, address patient inquiries, and resolve concerns promptly.
f) Provide clinical guidance and leadership to staff within the unit. Ensure compliance with established clinical protocols, safety standards, and the delivery of high-quality care. - Supports Staff Development and Professional Growth
g) Fosters the clinical competence of staff through targeted development initiatives.
h) Serves as an on-site resource for clinical team members, offering guidance on clinical issues and problem resolution.
i) Implements changes to personnel, processes, and procedures to meet organizational and departmental objectives.
j) Provides input to the Manager, Post-Acute Services and Population Health, for interim and annual performance evaluations, focusing on clinical competence and observed performance.
k) Collaborates with the Manager, Post-Acute Services and Population Health, and staff preceptors to support the orientation process for new staff.
l) Continuously evaluates staff performance, offering constructive feedback to enhance clinical outcomes.
m) Supervise, mentor, and support staff. Holds staff accountable for adhering to national care management standards of practice.
n) Facilitate ongoing education and training for staff to enhance their clinical skills and knowledge. Stay updated on the latest developments in healthcare and clinical practices.
o) Lead quality improvement initiatives within the unit. Participate in audits and compliance efforts, identify areas for enhancement, and implement corrective actions to improve patient outcomes. - Provides Leadership to Ensure Operational Effectiveness
p) Regularly meets with the Manager, Post-Acute Services and Population Health to discuss concerns and formulate resolutions.
q) Recommends strategies for improving workflow efficiency and clinical performance.
r) Collaborates with the manager to implement staffing schedules and daily assignments that align with patient care needs and budget constraints.
s) Monitors staff attendance and manages PLAWA and paid time off (PTO) in compliance with organizational policies.
t) Assigns patients to nursing and social work case managers to ensure continuity of care within the integrated model of care management services (dyad approach).
u) Efficiently manage unit resources, including staffing, supplies, equipment, and budget allocation. Monitor and control expenses while maintaining high care standards.
- Bachelors Degree Required
- Masters Degree Preferred
- Clinical background including nursing, social work, or another clinically-related healthcare field or similar advanced degree required. And
- 3 Years Minimum 3 years of quality management/care coordination/health care experience
- Strategic thinker with experience in a value-based care management environment;
Care Management/Utilization Review experience with CMSA or ACMA certification preferred. - Experience working with physician colleagues to build and advance clinical programs
