• Coordinate the development of patient care plans and ensure accurate and timely completion of MDS assessments
• Collaborate with interdisciplinary team members to identify patient care needs and develop effective care strategies
• Maintain accurate and up-to-date medical records and reports
• Conduct thorough assessments and evaluations to identify patient needs and develop personalized care plans
• Develop and implement education programs for patients and families to promote health and wellness
• Provide ongoing monitoring and evaluation of patient progress, identifying areas for improvement and implementing changes as needed
• Participate in quality improvement initiatives to improve patient outcomes and reduce readmissions

• Current RN licensure in the state of Ohio
• 1+ years of experience in a similar role
• Strong knowledge of MDS assessment tools and regulations
• Excellent communication, organizational, and problem-solving skills
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
• Strong attention to detail and accuracy in record-keeping
