Your Role: As a vital member of our interdisciplinary team, you’ll work under the supervision of a Clinical Manager to coordinate all aspects of patient care—ensuring individualized, compassionate support that maximizes quality of life.

Key Responsibilities:

• Conduct ongoing, holistic assessments of patients and families
• Develop and manage comprehensive care plans in coordination with the care team
• Facilitate interdisciplinary case conferences and care planning discussions
• Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support
• Advocate for patients and families, addressing physical, emotional, and psychosocial needs

What You’ll Bring:

• Active Massachusetts RN license in good standing
• Associate’s degree in Nursing required; BSN or equivalent preferred
• Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
• Strong interpersonal skills and the ability to thrive in a collaborative team environment
• Flexibility, self-direction, and sound clinical judgment
• Valid driver’s license and access to an insured, reliable vehicle

Pay Range

$42 - $56 USD

Benefits are offered to employees that are scheduled to work 20+ hours/week, which include a generous earned time (vacation days) program, tuition reimbursement, student loan paydown, two retirement plans, medical/dental/vision/life/disability insurance, and more.
