• Experienced LCSW passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery.

• Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
• Set measurable objectives that are formulated in conjunction with the rehab team.
• Assist the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems.
• Instruct or counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
• Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs.
• Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives.

• Master’s Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker licensure in the state of practice.
• Minimum one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis.
• Current BLS (Basic Life Support), First Aid and CPR certification required.
• Ability to lift 50 pounds; lifting or transferring patients up to 100 pounds following safety procedures.
• Fine motor skills, visual acuity, and walking/standing for extended periods.
• Physical tasks include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.

USD $50.00 - $55.00 / Hour
