Job Description:Job Title: Social Worker – Learning Disabilities
Location: Walsall
Pay Rate: £28.40 per hour
About the Role:
Liquid Personnel is recruiting Adult Social Workers to join its client’s Learning Disabilities Team based in Walsall. In this role, you will support adults (18+) with a diagnosed learning disability to lead safe, independent, and fulfilling lives.
This is aperson-centredandstrengths-basedrole, requiring collaboration with multiple agencies to meet care and support needs in line with theCare Act 2014.
What will your responsibilities be?
In this role, you will
- Complete Care Act assessments to determine eligible care and support needs for patients ready for discharge.
- Develop and implement strengths-based support plansin collaboration with the individual, their family, and professionals.
- Work effectively within amulti-agency team including hospital discharge coordinators, nurses, OTs, and physiotherapists.
- Undertake safeguarding enquiries under Section 42 where appropriate, ensuring adult protection procedures are followed.
- Carry out Mental Capacity Act assessments, and contribute tobest interest decisions when capacity is in question.
- Support timely discharge planning, ensuring that delayed transfers of care are minimised.
- Respond tourgent or unplanned referrals via the duty system, triaging and prioritising cases based on presenting risk.
- Promote the use of community-based resources,reablement, and preventative services to avoid unnecessary readmission.
- Maintain high-quality, accurate, and timely case records using systems such as Mosaic.
- Contribute to regular reviews and reassessments to ensure that support remains proportionate and effective.
Qualifications & Experience
- Professional
Qualification:
MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW. - Registration:
Must be registered with Social Work England. - Experience:
Substantial experience working with vulnerable adults, including those with complex needs. - Legislation Knowledge:
Strong understanding of theCare Act 2014,Mental Capacity Act 2005, and safeguarding procedures. - Skills:
Excellent assessment, analysis, and communication skills. Ability to work collaboratively in a multi-disciplinary setting. - Driving Licence:
Full UK driving licence and access to a car is often required for community visits. - Free DBS and compliance service
