Position: Clinical Lead Nurse Single Point of Access (SPA)

Overview

The post holder will be part of a dedicated Single Point of Access team and will support the delivery of the Healthy Child Programme (HCP) 0-19. The SPA operates a text, telephone and email service to children, young people, parents/carers and other professionals. The team offers advice and support, and provides information and sign posting. We may close this vacancy early if we receive high numbers of applications.

The service is led by Specialist Community Public Health Nurses (Health Visitors/School Nurses) who provide expert advice, support and interventions to children and families, supported by a skill mix of clinical and administrative staff.

Responsibilities
  • To coordinate a team of nurses, assistant practitioners and administrative staff to provide a high quality clinical care programme via the Single Point of Access
  • To provide triage, clinical assessment and advice, sign posting to support for children, young people and families who are accessing the Healthy Child Programme 0-19
  • To provide timely and relevant information to safeguarding children and young people, working within local safeguarding policies and procedures.
  • To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience, ensuring the highest standard.
  • Have a good sound knowledge of Children’s Public Health agenda and Healthy Child Programme 0-19

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. The Trust includes the Manor Hospital with facilities such as a Critical Care Unit, Neonatal Unit, Obstetric Theatre and Integrated Assessment Unit.

A new Urgent and Emergency Care Centre opened in March 2023, significantly improving emergency care facilities and capacity.

Responsibilities (continued)
  • To be responsible for the triage and assessment of care needs of children, young people and parents/carers who contact the SPA.
  • Demonstrate a high level of judgmental/analytical skills when interpreting and performing assessments and care planning via the telephone, text and virtual consultations.
  • Provide consultations and information (verbal and/or written reports) to other professionals who need advice and guidance about children and young people’s health and wellbeing, working within policies to maintain patient confidentiality, child and adult safeguarding and information sharing.
  • Maintain accurate and up-to-date nursing records, maintaining patient confidentiality, and statistical records as required by the service.
  • Work with other professionals to promote multidisciplinary / multiagency working.
  • Be able to utilise the service pathways of care, sign post and/or refer to multi-agency pathways and services and/or escalate concerns when appropriate, including multi-agency liaison.
  • Coordinate and have oversight of daily workload of SPA, showing appropriate time management.
  • Provide excellent evidence-based clinical triage.
  • Identify poor professional practice and take appropriate action to address it.
  • Act in a professional manner at all times whilst on Walsall Healthcare NHS Trust’s business and demonstrate excellence in clinical practice, ensuring compliance to Trust policies and procedures.
  • Assist the senior leadership team in strategies that promote quality assurance, Clinical Governance and quality audit.
  • Develop clinical skills and access training as required / directed to enable participation in future service developments.
  • Assist in the supervision of qualified and unqualified staff and facilitate their continuing professional development.
  • Ensure effective learning experiences and opportunities to achieve learning outcomes for students and the provision of learning opportunities.
  • Communicate information to children, young people and parents/carers with empathy and reassurance.
  • To ensure that patients and users of the service are involved and to assist in providing links with the public and PALs (e.g., to pick up on the themes of complaints and report clinical concerns).
  • To participate in service user forums.
  • To maintain a patient-led focus in all aspects of care and co-ordination.
