Role Overview

We are recruiting qualified Social Workers on behalf of a local authority in the West Midlands to support Adult Social Care – Intermediate Care Services
. This role is offered on a hybrid basis, with onsite working across community and hospital settings. This is a contract opportunity providing cover for a vacant post within a busy and well-established service.

Key Responsibilities

• Managing adult social care caseloads within Intermediate Care Services
• Completing assessments, reviews, and care planning
• Supporting hospital discharge and community-based interventions
• Working collaboratively with health professionals and multidisciplinary teams
• Ensuring safeguarding, compliance, and statutory responsibilities are met
• Qualified Social Worker
• Current Social Work England registration
• Demonstrable experience within Adult Social Care
• Full UK driving licence (essential)
• Ability to work on a hybrid basis
• Willingness to work Inside IR35
