Job Summary

With 114+ years of service to the community, the Natick Walpole Visiting Nurse Association is one of the most trusted non-profit providers of home health, public health, and community- based certified health home care in the region.

We are looking for a compassionate, reliable PT to join our team to provide excellent care and who enjoys working with patients in their homes.

Openings for 2 to 3 Physical Therapists for coverage areas for Natick, Ashland, Framingham, Wayland, Weston, Wellesley, Needham, Dover, Sherborn

Responsibilities
  • Provide care and services in accordance with the plan of care.
  • Assist community members and health field personnel to assess, plan for, and provide needed health and related services.
  • Document care provided in an accurate and timely manner.
  • Provide teaching to clients and families.
  • Coordinate and collaborate with other members of the health care team, community agencies, and referral sources.
  • Communicate the plan of care with patients and families and ensure understanding of the exercise program prescribed.
  • Responsive to meeting or exceeding patient/client expectations and promotes a positive image of NWVNA.
  • Adhere to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
  • Attend continuing education programs.
  • Other duties assigned by the department manager.
Requirements/Qualifications
  • Current licensure from the Board of Allied Health in the State of Massachusetts
  • Graduate of an accredited college of physical therapy
  • Must have demonstrated competence in community health or clinical patient care setting, providing direct patient care with patients for a minimum of two years. Associate’s degree and graduation from an accredited Physical Therapy program.
  • Demonstrates responsibility for self-direction
  • Demonstrates the ability to learn and utilize computer technology
  • The ability to work in a team-oriented environment as well as independently
