Acute Care Nurse Practitioner

Overview

GHR Healthcare is partnered with an organization with an 'Advanced Care at Home' model seeking a Physician Assistant/Acute Care Nurse Practitioner with a Hospitalist background to support them on a full-time, long-term contract.

Advanced Care at Home –
Locum Tenens ACNP or PA-C –
Walnut Creek, CA

The APP is responsible for providing high-quality patient care to various patient populations. They serve as the point of contact representing the Acute Care at Home program to the patients and families, collaborating with nursing staff and multidisciplinary care team. The APP also works collaboratively to identify patients who are appropriate for the program, facilitate enrollment, and develop and implement an acute plan of care in the home.

Contract Duration: Contract will be renewed annually

Schedule: 10-hour shifts, 7 days on 7 days off
, 8a-6p PST

Compensation:

• $125/hr
• $1500 CME allowance per year
• Reimbursement for ACLS, BLS, and License renewals
• Reimbursement for miles and tolls - with receipts and maps submitted weekly
