Job: Occupational Therapist (OT) Home Health - Full Time / Part Time

Location: Walnut Creek, CA

ABL Health Care and ABL Hospice provide home healthcare and hospice services dedicated to compassionate, personalized care that prioritizes comfort and dignity for patients and families. We are seeking a compassionate, organized, and team‑oriented Occupational Therapist (OT) to join our Home Health team in Walnut Creek, CA, as a Full Time or Part Time (Per Diem).

As an OT with ABL Health Care, you will evaluate patients, create and implement care plans, and track progress, working closely with the Clinical Director and Clinical Staff both remotely and in the field.

• Provide, coordinate, and oversee skilled intermittent care to patients, including instruction and education of patients and/or caregivers to promote maximal functional status, in the patient s home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to the Physician, Case Manager, and/or Director of Nursing as needed.
• Initiate, develop, implement, and revise the patient s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs related to the patient s condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

• Current OT License to practice in California — Required
• Experience in Home Health or Hospice care — 1 year required
• Current CA Driver s License
• Able to travel within a 15–25 mile radius on a regular basis
