Overview
Elevance Health's Carelon Behavioral Health is seeking a Wellness & Recovery Specialist in California. The role focuses on care coordination and management related to member wellness and recovery. Candidates must be California residents and have a valid Peer Specialist certification.
ScheduleMonday‑Friday, 8:00 a.m.‑5:00 p.m. PST.
Employment TypeVirtual full‑time, with required in‑person training sessions. Candidates not within a reasonable commuting distance will not be considered unless an accommodation is granted as required by law.
Responsibilities- Identify opportunities for member and family engagement in a supportive recovery network.
- Develop and implement onsite psychiatric discharge planning education at Recovery and Resiliency sites.
- Collaborate with Stabilization Teams as a member advocate in discharge planning education, resolving barriers, and coordinating transition of services.
- Act as a resource for staff on decision‑making and problem‑solving.
- Initiate and maintain contact with assigned individuals and providers to assess member response to services.
- Associate's degree (AA/AS) and minimum 2 years of experience in health services, behavioral health, or peer support services, or an equivalent combination of education and experience.
- Peer Specialist Certification is required.
- Knowledge of care‑coordination and case‑management concepts.
- BA/BS or MBA preferred.
- Comfortable managing high call volume.
Hourly rate: $22.98 – $37.61, depending on location, experience, and skills.
