Traveling Physical Therapist — Full Time — Sonoma

Location: Sonoma, CA

Pay Range: $72.00 per hour – $80.75 per hour (actual pay based on skills and experience).

Bridge Home Health & Hospice is looking for a skilled Physical Therapist who thrives in new environments and enjoys bringing clinical impact to communities across sunny California. This unique internal travel role offers guaranteed 40 hours per week and 13‑week assignments across our network of Bridge Home Health locations. You will receive a housing and food stipend, full benefits, and the stability of a direct employee position—not a third‑party travel contract.

Candidates must be based in California or willing to relocate.

Responsibilities
  • Provide physical therapy services to patients according to a written physician’s plan of care, as defined in the state Physical Therapy Practice Act.
  • Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
  • Evaluate the home environment and assess muscle strength, mobility, gait, ROM, and potential for rehabilitation.
  • Develop and direct the physical therapy treatment program.
  • Instruct patients and family/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy and report patient responses to the physician.
  • Initiate physical therapy programs and instruct other personnel and family members in certain phases of therapy.
  • Train patients in the use of prosthetic devices when required.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on attainment of goals.
  • Participate in discharge planning and in-service education programs for nursing organization personnel as needed.
  • Participate in peer consultation processes and supervise physical therapy assistants per organizational policy and state regulations.
  • When physical therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
Qualifications
  • Doctor of Physical Therapy (DPT) from a program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  • Licensed to practice as a physical therapist by the Physical Therapy Board of California.
  • Good verbal and written communication and organizational skills.
  • Current CPR certification.
  • Valid driver’s license with an automobile in good working order and current auto insurance.
  • Residence must be 55 miles or more from the work location.
  • Reliable transportation.
Physical Requirements
  • Occasional sitting for 1–3 hours.
  • Frequently standing for 3–6 hours.
  • Occasional driving or foot control for 1–3 hours.
  • Occasional walking on level surfaces for 1–3 hours.
  • Intermittent walking on uneven surfaces for less than 1 hour.
  • Intermittent bending or climbing for less than 1 hour.
  • Occasional twisting, kneeling, crouching, and crawling for 1–3 hours.
  • Intermittent lifting of up to 50 pounds for less than 1 hour.
  • Intermittent carrying, pushing, or pulling of up to 50 pounds for less than 1 hour.
