Nurses at Home is a home health company looking for a full‑time RN Clinical Case Manager. Former home‑care experience and proficiency with OASIS are preferred. The role requires a positive attitude, proficiency, and a warm heart. It offers top‑of‑industry rates and opportunities for growth.

• Provide clinical case management for home health patients.
• Manage OASIS documentation and ensure compliance.
• Collaborate with owners and clinical staff to enhance patient care.
• Maintain patient independence and support functional goals.
• Participate in on‑call weekend rotation and standard Monday‑Friday schedule.

• Registered Nurse (RN) – required.
• Bachelor’s degree – preferred.
• 2 years of RN experience – preferred.
• Home health experience – preferred.
• Proficiency with OASIS documentation.
• Strong communication, empathy, and problem‑solving skills.
• English – required.

• Monday to Friday.
• Weekend on‑call rotation.
• Work on the road.

• Two weeks PTO.
